Director of Care Coordination

The Director is responsible for providing strategic leadership and administration related to patient care, education, safety and quality in accordance with organizational policies and professional clinical practice. Promotes a professional practice environment through collegial and collaborative partnerships with all stakeholders across the system regarding clinical operations, education, customer service, patient safety, financial stewardship, quality outcomes and regulatory compliance.

Qualifications:
• Bachelor’s Degree; Master’s Degree within 2 years of hire into the position
• 5 years in the specialty area
• 3 years as a leader in the specialty area; experience in systems level leadership
• Current DC state license and BLS certification required
• Certification in leadership or specialty within one year of hire into the position
• Actively participates in professional organizations

Responsibilities:
• Role models positive customer service behaviors through the consistent application & deployment of HUH’s Values and Mission principles and holds self & others accountable
• Establishes strategic plan, provides resources and has a clear vision for specialty/service line
• Promotes cohesive, intra and inter-disciplinary work teams
• Models & encourages professional practice that includes autonomy, authority, accountability & responsibility while honoring a diversity of opinion, the value of recognition, demonstrating respect for all contributors, and resolving issues supporting a “just culture” perspective
• Oversees development & implementation of care delivery model, establishes clear expectations
• Advocates for resources for safe delivery of patient care related to labor, equipment & facilities in specialty area with a global perspective
• Embraces and cultivates an environment of life-long learning for self and entire staff
• Executes all other duties as required
