BASIC FUNCTION :
Assess patient and family psychosocial and discharge planning issues relevant to medical treatment. Provides crisis intervention, emotional support, resource information and discharge planning. Arranges case conferences, facilitates bioethics consultations.
 
NATURE AND SCOPE :
Makes social work services available to all customers, regardless of age, of Howard University Hospital. Serves as the coordinator for discharge planning on the assigned unit. Assists patients and/or families in coping with illness, hospitalization and continuing care needs.
 
PRINCIPAL ACCOUNTABILITIES:
  • Performs psychosocial assessments and discharge planning for high risk patients.
  • Participates in patient care planning responsive to age specific needs.
  • Works independently within the assigned area and is accountable for the development and maintenance of service therein. Is responsible for seeking consultation when needed and keeping supervisor apprised of patient/program developments. Work is reviewed through medical chart documentation, reports, conferences and feedback from other disciplines.
  • Utilizes crisis intervention and individual or group counseling techniques to assist patient and family in coping with stress of illness, hospitalization and lifestyles changes.
  • Facilitates communication between the patient, family and health care team.
